Due to COVID-19, a lot of changes have had to happen in sporting events and because of this, it is mandatory for you to watch the Force Health Protection Brief video below and complete the self-declaration prior to the event. Submission of the Self-declaration form needs to be submitted 24hrs before the event, by simply completing the below form.
